Enter Learning Assessment & Evaluation Form

There are two choices for completing the learning assessment and evaluation form:

• Complete both the learning assessment and evaluation form online
  
  
• Complete the learning assessment online, and then print and mail or fax the evaluation form
  (Adobe Acrobat Reader version 5.0 or higher required)

Upon successful completion of the entire program, post-test (grade of 70% or higher) and submission of the activity evaluation, a certificate of completion will be issued to you via email or US mail within 30 days.
